Request Appointment

The best way to schedule an appointment is by calling our main office number, 970-495-0555, during our office hours:

  • Monday: 9:00 am – 4:30 pm
  • Tuesday: 9:00 am – 4:30 pm

However, for your convenience in scheduling appointments, we accept requests through our online scheduling form in addition to over the telephone.  Please note that appointment times are subject to availability, though we often have same day MD appointments available if you call us first thing in the morning.

If it is after hours or you would prefer to request an appointment online, please fill out the following form and our friendly staff will contact you to confirm your request or find an available time that suits your schedule.

Remember…
